Much sympathy extended to officer's family and sadness for officer.
Of course don't know, but first thought based upon experience from my location and knowing the perpetrating driver was in a mini van - likely a woman talking on the cell phone. Encounter them on every ride. Very dangerous and discourteous drivers.
Good advice given in OP. - Cover brakes at intersections
- High beams on during daylight hours
- Add more lighting front and rear
- Pipes to legal limit (and a bit beyond IMO)
- Safety gear - I respect your right to choose.
Even with the above, the motor officer had all the above, it did not protect against a discourteous and very bad driver. 
Bring back the pillory.

Condolences to the family & friends for their loss. Despite our best efforts and preparation the reality is it could happen to any one of us. I am a believer in lights (front & rear) and also run on cruise frequently with the front brake covered in city driving.
Look at front wheel of target if possible
If front wheel is not moving make eye contact with driver
& even this cannot protect you if their mind is not focused on the task at hand (driving a two ton brick wall).
I know guys who have ridden for 50+ years without an incident & others that can't seem to get out of their driveway without going down.
I have been very fortunate to have only laid one down with no injuries in 30+ years of riding and many 100k's under my belt.
06 SE Ultra 93k on the clock
10 Ultra classic LTD 72k on the clock
12 FLTR 9 k on the clock in three months
I have two sons that ride ~ my advice to them was
1. Where you look, you go
2. Their job is to RUN you over & your job is to get home safe. We cannot take it personal, accidents are just an unfortunate part of all drivers/riders reality.

This sucks and brings back recent memories of my buddy who died as a result of a failure to yield wreck. In that case, the pickup driver received 2nd degree vehicular homicide charges. I hope the driver in Officer Yung's case will be held accountable in a similar way.

Always sad to hear of something like this, rest in peace, Officer Yung. Very dangerous riding a bike around DC area, some of the worst drivers I've ever seen. As others have said, defensive driving is your best bet. Better lighting helps to an extent, but I've seen firsthand responding to incidents in a huge ass red truck with countless lights, sirens and air horns blowing, etc and people still find a way of not seeing you and pulling out. If you think dozens of lights and wearing a road crew reflective vest makes you safer, go ahead. I personally think that it gives riders a false sense of security. Nothing is fool proof, but I use what lights they gave me and keep my eyes everywhere but in front of me for the most part.
Ride safe everyone.
