Question Front disc eating wave washers

Hey all, My bike seems to eat wave washers on the right front disc.
INFO: 2017 FLTRU - 26k miles.
Front tire changed at 23.5k miles at a dealer. Within 100 miles I started hearing a clunking at low speed and over bumps in the road. When I would push the bike it would clunk consistently.
Changed the brake pads and found the right front rotor was loose. Pulled the bolts and bushings and found the wave washer was missing at all five points.
Replaced the wave washers and all was good. No clunk at bumps or slow speed.
Just turned 26k and the clunk returned. Found all five wave washers were gone again.
So the questions are; Is this a balance issue, or is there another cause relating the the wheel, hardware, disc, etc...? Why only one disc?
True the washers are 78 cents apiece but still a pain every 2k miles plus the extra wear on the disc from flopping around.
Any thoughts or advice would be appreciated. Thanks.

@BigBlockBobby - Thanks for the reply. I was in the process of pulling it apart to do just that when I found the spacer was in backwards and the right fork was not seated against the backwards spacer (photo in previous post). Put it all back together and replaced the washers for a third time. So far so good (knock wood).
