The top ball broke off the push rod, It looks like the push rod just pulled apart right at the bottom of where the ball pressed into the tube.

These are top of the line aluminum adjustable, I have about 7000 mile on them, stock motor.

Anyone know what could have caused this to happen

The word is aluminum.
We have witnessed aluminum pushrods fail in T/C's as well as Evo's.
Rev-Tech's to be exact, all were fubared at/near the rocker arm.
If trying to save valvetrain weight, that is not the area in which to do it in.
Install a high quality chome-moly pushrod such as an S&S, and sleep soundly.

We like to load the lifter .140-.150" to insure quiet valvetrain operation at full operating temps.
An adjustable pushrod will allow that to occur.

Aluminum pushrods in an Iron XL? Yes, they were OEM.
Aluminum pushrods in Pans/Shovels are ok, as long as the spring pressures are kept in check.
Aluminum pushrods in Evo, and T/C application, NO.
